- Old prescriptions and test reports, especially OCT and visual field reports
- The eye drops you are using, and your glasses
- A list of other medicines, and whether you have diabetes or blood pressure
- Insurance card and ID, if you have health insurance
- Someone to drive you home if your pupils are dilated with drops
